Repeat and Serious Violations OSHA initiated an investigation of a fire and smoke damper manufacturer as a follow-up to a comprehensive inspection conducted in 2005. As a result of that investigation, the company was cited for five willful and four repeat violations, plus one serious violation carrying proposed penalties totaling $140,760. The willful violations allege failure to provide elements of a hearing conservation program, including audiometric testing and training to employees exposed to noise levels above 85 decibels.
